病人 of 纽约医院 who are staying for inpatient care will have their treatment managed by one of our Hospitalist providers. The Hospitalist will direct the patient’s care in the hospital and are available to answer any questions from the patient and their family. Hospitalists work with the patient’s primary care provider to ensure any history or significant findings are discussed. Volunteers take time to chat and play cards or get a movie, book or favorite newspaper. Hospitalists are providers (physicians, nurse practitioners and physician assistants) who devote their practice to the care of hospitalized patients. They see patients who have been admitted to the hospital. They do not see patients outside the hospital.

- 放电信息: Your doctor and your nurse will provide you with instructions for your post-hospital care. If you have questions about your diet, activities or other matters, please feel free to ask. If you get home and realize you forgot to ask a question, don’t worry. We’re only a phone call away. Your discharge instructions will contain a phone number for you to call should you need anything.
- 药物: If your doctor gives you a prescription, you can take it to a local pharmacy, or it can be filled in one of the hospital’s retail pharmacies.

Our 家庭护理 service is a full-service Medicare-certified home care agency, 提供护士, 护士助手, medical social workers and physical, occupational and speech therapists for home health care in the hospital’s service area. We also offer clinical hospice services through a partnership with Merrimack Valley 临终关怀. Referrals for private duty nurses, 护士助手 and adult sitter services for discharged patients are also available through 家庭护理.

The Bridges Program is a volunteer program that offers seniors help with filling the gap in services after discharge from the hospital. The program offers services such as picking up prescriptions, 固定便餐, 超市购物, picking up the family pet and more.
